Incident Narrative

At approximately 11:30 on 15 September 2015, an employee was hospitalized when he was struck in the chest by a cap on a pressure vessel that he and another employee were removing. Remaining pressure in the pipe leading to the pressure vessel caused the cap to violently dislodge during the procedure and strike the employee in the chest, breaking one rib and causing the employee to suffer a broken left elbow, a laceration that required sutures on his right elbow and a hemorrhage on the artery leading to his adrenal gland.
