Incident Narrative

An employee was removing a sleeve that goes around a bar weighing approximately 50 to 100 pounds. The sleeve was secured with four bolts; three were partially removed and one was removed completely. As he turned the sleeve, his left ring finger got caught between the sleeve and bar, causing a partial amputation.
