Incident Narrative

An employee was preparing to drill a hole in wax paper using a foot-operated paper drill machine. She placed a stack of paper in the machine using a block to hold the wax paper in place. When she pressed the foot pedal, the machine drilled the tip of her right index finger, causing an amputation-type injury.
