Incident Narrative

An employee was doing a service check on a tilt bed trailer. The employee went to the front of the trailer to lock it in place so it wouldn't tilt. There was a small gap in the neck of trailer. A coworker stepped onto the back of the trailer and the trailer went upward, crushing and amputating the employee's finger.
