Incident Narrative

An employee on a ladder was drilling holes in the wall to run fiber optic wire for a breakroom television monitor. As he tried to pull the wire through the hole, the ladder slipped out from under him and slid down the wall it was leaning on. He landed on the ladder and the floor, fracturing his right femur.
