Incident Narrative
The employee returned to the cement plant and was washing the back of the mixer truck with the cement hose. The fold down ladder part fell down as the employee was rolling up the water hose striking the employee on the left hand and fracturing the left index finger. The employee was taken to the hospital and admitted.
