Incident Narrative

On August 5, 2015, at approximately 2:30 p.m., an employee was using plasma cutter to cut a length of 2-inch pipe. The pipe was on an automated table that uses rollers to move the pipe down the line and out of the building for offloading. Towards the end of the process, the employee pushed the pipe to speed up its exit from the building. His glove got caught by the roller, which pulled his arm in and trapped him between the roller and a metal bar. He fractured bones in his arm and hand that required surgery.
