Incident Narrative

An employee was using a polishing cloth to clean aluminum tubes after they were cut by the automatic circular saw. As the employee was moving aluminum tubing down the finishing table air that was blowing from a fan flipped a corner of the polishing cloth that was in his hand under the saw guard. The saw then pulled in the cloth and the employee's right index finger was amputated.
