On July 28, 2015 and approximately at 1:30 p.m., a welder was operating a milling machine in the machine shop to fabricate two sheet metal pieces for a printing press (screen printing industry). His left hand middle fingertip became caught in the two sheet metal pieces and a running drill bit of the equipment.
