Incident Narrative
An employee operating a rolling bar of a cold rolling machine (a machine which treads the metal bar) was injured during operation. The employee was running a previously treaded metal bar/bolt through the cold rolling machine; while standing on a one foot metal grading/platform. The employee's shirt got caught on the spinning bar, and as the employee tried to break free, the employee's pant leg got caught, thereby flipping the employee over the bar onto a concrete floor and breaking his leg.
