Incident Narrative
An employee was attempting to clear a blockage in a corn flaker. The equipment was shut down and locked out at the time. The employee removed a corn cob from the rollers when residual pressure from the rollers smashed the employee's fingertips, resulting in an amputation of the middle left-hand finger.
