Incident Narrative

An employee was operating a hydro blaster while cleaning a heat exchanger. As he exited a scaffold that was 15 to 20 feet in height, he slipped and fell through the ladder cage onto the concrete surface, injuring his lower back. He was hospitalized for surgery on his lower back.
