Incident Narrative
On or about 3/31/2015, at approximately 9:00 p.m., an employee's left arm got caught in a bowling pin resetting machine. The employee reached into the side of the machine to adjust a pin and the machine cycled. The machine was not de-energized at the time of the incident. The employee sustained soft tissue injuries and was hospitalized.
