Incident Narrative
An employee was conducting preventive maintenance with two other people. One person was responsible for turning the power switch for the motor of a rotating floor. While the motor was in operation, the employee's hand was near the pulley and his right middle finger was caught in the belt, resulting in an amputation at the top joint.
