Incident Narrative
On or about 1555 hours on March 10, 2015, the right intermediate and distal phalanges of an employee's middle finger were amputated after his finger got caught in the pulley of a winch he was operating. The employee was holding the winch cable with his right hand due to a kink in the line and operating the winch with his left hand when his hand was pulled into the pulley. He was hospitalized and the phalanges were surgically removed.
