Incident Narrative
A helper began raising the powered dock plate. While the dock plate was in motion, the injured employee walked onto it and shut the trailer door. He slipped, and got his legs stuck between the dock plate and the back of trailer. While he pulled up on the plate to get if off his legs, he told the helper to pull the truck out to free him. When the truck was pulled forward, the plate snapped downward and severed the ends of the helper's left index and middle fingers.
