Incident Narrative

A group of employees was using a crawler crane to lower a toolbox from the fourth floor down to the third floor. Once the toolbox was lowered, an employee unhooked the straps from the crane's hook and the ball shifted and pinched his right index finger between the hook and the ball. The employee's right index fingertip was amputated.
