Incident Narrative

Employee was loading boxes onto a conveyor belt when his glove got caught in the belt. As a result, the employee lost the tip of his left index finger. The worker was taken to a nearby clinic and was then transported to a Reconstructive Orthopedic Center located at 1200 Binz St. Houston, TX 77004.
