Incident Narrative
Employee was driving a standard roll-off truck for picking up containers of spent material from various metalworking companies in the area. He was on-site to pick up a metal box type container from a customer. The employee hooked the truck's wire rope cable and winch up to the box and engaged the winch briefly. The box was moved close to the bed of the truck and raised a few inches off the ground. The winch was disengaged. He then placed a tarp over the box. As the employee was placing the tarp, he stepped onto the chassis rail on the back of the truck. At this time, the hydraulic winch pulled the wire rope slightly. The employee's left foot was caught between the box and the back of the truck. Medical services were called. He was taken to a hospital. Several bones were broken in the left foot.
