Incident Narrative

The employee was performing maintenance on a piece of machinery next to a bag machine where film was being pulled. The bag machine started malfunctioning. The employee began to re-route the film on the bag machine while the equipment was energized. The employee got his left hand caught in the rollers of the bag machine, which pulled his hand into the gears and belt.
