Incident Narrative
A machine operator was using a grease gun to apply lubricant to the shovel area of the excavator. The Shovel was situated on the ground, and very close to the edge of an 8 foot drop on a pile of material. The employee lost his footing, fell down the 8 foot drop, and struck his head on a concrete object resulting in multiple broken ribs and a head injury.
