Incident Narrative
While making deliveries the vehicle's brakes failed causing the vehicle to roll. The driver tried to stop the vehicle by chocking the wheel. While attempting to chock the passenger side rear wheel the employee's left index finger was trapped between the metal chock and the tire resulting in an amputation.
