All data comes from the Centers for Medicare & Medicaid Services (CMS) Provider Data Catalog. CMS publishes quality ratings, deficiency counts, staffing data, and fine amounts for all 14,695 Medicare-certified nursing homes in the United States. Data is available through the CMS public API at data.cms.gov.
The overall 5-star rating is a composite of three separate ratings: Health Inspections (based on deficiencies found during state surveys), Staffing (based on registered nurse and total nurse hours per resident day), and Quality Measures (based on clinical outcomes like falls, pressure ulcers, and use of restraints). Each component is rated 1-5 stars and combined into the overall rating.
