FUEL SYSTEM, GASOLINE:DELIVERY:HOSES, LINES/PIPING, AND FITTINGS
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2007-2010 Lexus LS vehicles manufactured May 9, 2006, to July 20, 2010, 2006-2011 Lexus GS vehicles manufactured January 6, 2005, to September 10, 2010, and 2006-2011 Lexus IS vehicles manufactured August 30, 2005, to September 10, 2010. Due to anti-corrosion plating remnants, fuel may leak from where the fuel pressure sensor is attached to the fuel delivery pipe.
Remedy: Toyota will notify owners, and dealers will clean and reseal the fuel pressure sensor mounting area, free of charge. The recall began March 3, 2015. Owners may contact Toyota customer service at 1-800-331-4331.

TOYOTA IS RECALLING CERTAIN MODEL YEAR 2006 THROUGH 2008 LEXUS PASSENGER CARS MANUFACTURED FROM AUGUST 26, 2005 THROUGH AUGUST 4, 2008. MICRO-FOREIGN OBJECTS IN THE MATERIAL OF THE VALVE SPRING MAY DEGRADE THE STRENGTH OF THE VALVE SPRING, POSSIBLY CAUSING THE SPRING TO BREAK.
Remedy: TOYOTA WILL NOTIFY OWNERS AND DEALERS WILL REPAIR THE VEHICLES FREE OF CHARGE. THE SAFETY RECALL BEGAN ON SEPTEMBER 2, 2010. OWNERS MAY CONTACT LEXUS AT 1-800-255-3987.

The contact owns a 2008 Lexus LS460. The contact stated while driving 70 MPH and depressing the brake pedal, the message "VSC Brake Malfunction. ABS Needs Services. Contact Dealer" was displayed. The contact was able to drive to the residence. The vehicle was taken to an independent mechanic, where it was diagnosed that the actuator needed to be replaced. The part was replaced; however, the failure recurred. An unknown dealer was contacted and informed the contact that the vehicle was not under warranty. The vehicle was not repaired. The manufacturer was contacted and informed the contact that the extended warranty coverage for the failure had expired. The failure mileage was 69,000.

The contact owns a 2008 Lexus LS460. The contact stated that the vehicle would erroneously start without anyone being near the vehicle with the starter button not being pressed. The vehicle was taken to the local dealer who replaced the starter however, the failure persisted. The contact indicated that the starter had been replaced on four separate occasions for the same failure. In addition, while driving approximately 40 MPH, a message to "Turn Off Vehicle and Contact Dealer" was displayed and the brake pedal became difficult to depress causing the braking distance to be extended. The vehicle was taken to the local dealer who diagnosed that the brake actuator was faulty and needed to be replaced. The actuator was replaced and the failure was corrected. The manufacturer was not yet notified of the failure. The failure mileage was 94,000.

WHILE IN MOTION, BRAKES STARTED PULSATING AND "CLICKING", THEN BRAKE PEDAL FEEL GOT VERY FIRM AND HAD DIFFICULTY STOPPING, ALMOST RESULTING IN ACCIDENT. TOYOTA HAS RELEASED WARRANTY ENHANCEMENT PROGRAM FOR THE BRAKE ACTUATOR ASSEMBLY, BUT MY VEHICLE IS JUST BEYOND THE 10YR/150K MILE PERIOD. I FEEL THIS IS A SERIOUS SAFETY MATTER AND NHTSA SHOULD HAVE WORKED WITH TOYOTA ON A SAFETY RECALL REMEDY. IF BRAKES FAIL, THE CONSUMERS WILL BE SERIOUSLY INJURED OR DIE. VEHICLE ONLY HAS 60K MILES AND THIS IS A KNOWN DEFECT!

TOYOTA ISSUED A WARRANTY ENHANCEMENT BULLETIN COVERED BY THE CUSTOMER SUPPORT PROGRAM ZE6 BUT ONLY INCLUDED THE 2007 LS460. HAVING DONE RESEARCH, OTHER OWNERS WITH THE SAME 2008 LS460 MODEL THROUGH THE 2014 LS460 MODELS ARE HAVING THE SAME ISSUE. TOYOTA IS REFUSING TO COVER THE REPAIRS AS "NOT ENOUGH COMPLAINTS HAVE BEEN FILED". MY VEHICLES IN USE DATE WAS SEPTEMBER 23, 2008. I WAS TOLD IN EARLY JUNE 2018 BY A LOCAL DEALERSHIP SERVICE EMPLOYEE MY CAR WOULD HAVE THE TRIM PANELS, CENTER CONSOLE AND DASH REPLACED BUT WHEN I CALLED LEXUS CARE I WAS TOLD IT WAS NOT INCLUDED AND WOULD NOT BE COVERED AS "NOT ENOUGH CLAIMS HAVE BEEN FILED FOR MY YEAR MODEL". SEEING AS HOW TOYOTA REPLACED THEM ON THE 2007 MODELS, THEY SHOULD BE REQUIRED TO ISSUE A RECALL FOR THE REMAINING YEAR MODELS THAT ARE HAVING THE SAME ISSUES.

I PUT MY 2 YEAR OLD IN THE CAR SEAT, STRAPPED HER IN, PUT MY BAG (WITH THE KEY FOB) ON THE BACKSEAT AND AFTER VISUALLY CONFIRMING THAT THE DRIVERS DOOR IS UNLOCKED CLOSED THE BACK DOOR WITH MY CHILD INSIDE. IN THE TIME IT TOOK ME TO WALK AROUND THE CAR TO THE DRIVER'S SEAT ALL FOUR DOORS SOMEHOW LOCKED DOWN. WITH BOTH THE KEY FOB AND MY CHILD INSIDE THE CAR; IN THE MIDDLE OF THE SUMMER IN TEXAS. LEXUS IS SUPPOSED TO HAVE A SAFE LOCK FEATURE THAT PREVENTS THE CAR LOCKING WHILE KEYS ARE INSIDE BUT THIS CLEARLY FAILED. LUCKILY WE WERE PARKED IN A GARAGE AND MY HUSBAND WAS ABLE TO COME IN 20 MIN WITH THE SPARE KEY; OTHERWISE TIT WOULD HAVE BEEN MUCH WORSE.

I WAS PARKED WITH THE ENGINE RUNNING FOR APPROXIMATELY 18 MINUTES WHEN SUDDENLY, I HEARD A BEEPING SOUND IN THE CAR IT WOULD NOT STOP!!! I TURNED MY ENGINE OFF IN HOPE TO RESET WHATEVER IT WAS BUT AS SOON AS ME FOOT TOUCHED THE BRAKE THE BEEPING RESUMED. IT WAS RAINING AND I HAD NOT CLUE OF WHAT IT WAS. AT THIS POINT, ALL OF THE BRAKE LIGHT ON MY DASH CAME ON . I WENT OUT OF THE CAR POPPED THE HOOD AND DIDN'T IMMEDIATELY RECOGNIZE ANYTHING. I THEN WENT BACK IN THE CAR AND WITH THE BEEPING STILL IN MY EAR, DECIDED TO HEAD HOME. THAT'S WHEN I NOTICED I COULD NOT STOP MY CAR !!! THE BRAKE WAS STIFF AND I LITERALLY HAD TO FORCE MY CAR TO STOP BY APPLYING AS MUCH PRESSURE AS PHYSICALLY POSSIBLE TO MY BRAKE PEDAL. I TOOK THE CAR TO MY MECHANIC AND AFTER DIAGNOSTICS HE STATED IT'S THE ABS PUMP AND BRAKE ACTUATOR.

I HAVE 2008 LEXUS LS 460 WHICH HAS MILEAGE OF 36,345. I HAD A SUDDEN FAILURE OF BRAKE SYSTEM AND WARNING LIGHT SAID "BRING THE CAR TO THE DEALER" THEY FOUND ABS ACTUATOR FAILURE. LEXUS ISSUED "WARRANTY ENHANCEMENT" ABOUT THIS PROBLEM IN 2017. BUT THEY WOULD NOT COVER MY CAR BECAUSE THE WARRANTY ENHANCEMENT EXPIRED 5 MONTHS AGO IN SPITE OF MY CAR'S LOW MILEAGE. I THINK NHTSA SHOULD RECOMMEND "RECALL" TO LEXUS BECAUSE SERIOUS SAFETY CONCERN OF THIS PROBLEM. APPARENTLY MANY OF THE OWNERS OF THIS VEHICLE HAVE HAD THIS PROBLEMS BUT HAD A LOT OF PROBLEMS WITH THE DEALERSHIP TO FIX IT. THE DEALER (LEXUS OF PALM BEACH, 5700 OKEECHOBEE BLVD, WEST PALM BEACH, FL 33417, (561) 291-6004) INSPECTED THE VEHICLE, CLEARED THE CODES, AND INDICATED THAT THE BRAKE ACTUATOR ASSEMBLY NEEDED TO BE REPLACED. THE DEALER AND THE MANUFACTURER INDICATED THAT THE VEHICLE HAD BEEN INCLUDED IN A TEN YEAR WARRANTY; HOWEVER, DUE TO A STATUTE TIMEFRAME, THE VEHICLE WAS CURRENTLY EXCLUDED. THE MANUFACTURER SUGGESTED REPLACING THE PART. THE APPROXIMATE FAILURE MILEAGE WAS 64,000.

DRIVER SIDE FRONT WINDOW HAS A WHISTLING NOISE THAT IS A HAZARD DUE TO THE DB LEVEL AT 55 MPH. I HAVE SPOKEN SEVERAL TIMES WITH CLEAR LAKE LEXUS AND THEY HAVE NOT OFFERED A SOLUTION. THIS APPEARS TO BE A VERY COMMON ISSUE WITH THE 2008 LEXUS 460 (LEXUS TALK). THERE APPEARS TO BE A FIX BY INSTALLING A TRIANGLE BETWEEN THE MIRROR AND DOOR.

ANYTHING THAT IS MADE OF THAT MATERIAL IS STICKY AND GETS THIS WEIRD BLACK STUFF EVERYWHERE. THE SHINY REFLECTIVE SURFACE THAT THE DASHBOARD IS A SAFETY HAZARD FOR MYSELF AND OTHER DRIVERS. WHILE BRAKING GRADUALLY, WITH THE BRAKE PEDAL IN A STEADY POSITION, THE CAR LURCHES/PULSES, THE TIRES MAKES LOUD SKIDDING SOUND THEN THE CAR COMES TO A HARD STOP AS THOUGH IT HAS SLAMMED INTO SOMETHING. THIS HAS HAPPENED ON MULTIPLE OCCASIONS AT LOW SPEEDS, WHERE IT FEELS LIKE IM PRESSING THE BRAKE AND LETTING GO, AND PRESSING AGAIN, BUT IN REALITY, THE BRAKES ARE BEING PRESSED AT A CONSTANT PRESSURE. THIS IS DIFFERENT THAN THE ABS FEELING, AS THE BRAKE PEDAL DOES NOT TRAVEL BENEATH MY FOOT, AND THE RELEASE AND GRABBING OF THE BRAKES SEEM VERY SLOW. IT HAPPEN THE FIRST TIME WITH ABOUT 50,000 MILES, GOING ABOUT 25-30 MPH. THEN WITH 86,000, GOING ABOUT 5-7 MPH.

THE BRAKES HAVE A CHATTERING NOISE AND FEEL THROUGH THE PEDAL WHILE STOPPED, WHEN YOU RELEASE THEM. THE CAR CAN BE IN PARK, AND IT STILL DOES IT. IT'S A KNOWN PROBLEM, BUT TOYOTA IS DOING THEIR BEST TO AVOID REPAIRING IT UNDER THE NEWLY EXTENDED WARRANTY ON THE "BRAKE ACTUATOR". IT'S 100% REPEATABLE, AS MANY TIMES AS YOU LIKE, IF THE ENGINE IS RUNNING AND THE BRAKE PEDAL IS PUSHED AND RELEASED.

WHILE DRIVING AT HIGHWAY SPEEDS (70 MPH) I ENCOUNTERED ILLUMINATION OF BRAKE/ABS WARNING LIGHTS ON THE DASH ADVISING ME THAT THE SAFETY BRAKE/ABS/TRACTION FEATURES ON MY CAR ARE NON OPERATIONAL. UPON PULLING OVER, I READ A MESSAGE DISPLAYED ON THE DASH ALERTING ME TO TAKE MY CAR TO THE NEAREST LEXUS DEALER. UPON ARRIVAL AT THE LEXUS DEALERSHIP, I WAS ADVISED BY THE SERVICE ADVISOR THAT THIS IS A COMMON PROBLEM ON THIS MODEL AND MORE LIKELY THEN NOT THE FAULTY BRAKE ACTUATOR WILL NEED TO BE REPLACED. THIS WAS CONFIRMED THE FOLLOWING BUSINESS DAY WHEN I RECEIVED A CALL FROM THE SERVICE ADVISOR CONFIRMING THAT THE BRAKE ACTUATOR HAS FAILED. UPON QUESTIONING THE ADVISOR OF THE CAUSE, HE ONCE AGAIN ACKNOWLEDGED THAT THIS IS A COMMON PROBLEM AND THAT LEXUS HAS ISSUED AN UPDATED PART AS A REPLACEMENT. IT CONCERNS ME THAT LEXUS HAS NOT PERFORMED A VOLUNTARY RECALL ON SAID BRAKE ACTUATOR SINCE THIS IS A CRITICAL SAFETY DEVICE AND THEY (LEXUS) ACKNOWLEDGE THAT THERE IS DEFECT TO THE ORIGINAL PART BY PROVIDING AN "IMPROVED" BRAKE ACTUATOR ASSEMBLY. FURTHERMORE, BY PERFORMING A SIMPLE INTERNET SEARCH I ENCOUNTERED NUMEROUS TOPICS ABOUT IDENTICAL ISSUES AS MINE ON THE MODEL/YEAR (2007-2010 LEXUS LS460). I WOULD LIKE TO DULY REQUEST THAT NTSB PERFORMS AN INQUIRY WITH LEXUS REGARDING BRAKE ACTUATOR ISSUES STATED ABOVE.

WHEN DRIVING ON A HIGHWAY, WARNING LIGHTS "CHECK ABS" "CHECK VSC SYSTEM" "CHECK BRAKE SYSTEM" "BRAKE MALFUNCTION" "STOP THE VEHICLE IN A SAFE PLACE IMMEDIATELY. CONTACT A DEALER" SUDDENLY CAME ON AND A 5 OTHER LIGHTS ALSO CAME UP. IT WAS SCARY ME SO MUCH. I HAD TO PULL OVER AND STOP. AFTER CHECKING TO SURE THE BRAKE STILL FUNCTION, I SLOWLY DRIVE HOME WITH THE EMERGENCY LIGHT ON. CAME HOME, I DID A LITTLE RESEARCH ON THE INTERNET, AND IT TURNED OUT THIS IS A KNOWN AND VERY COMMON PROBLEM FOR THIS CAR, AND LEXUS CONTINUE TO DENY THE PROBLEM, AND FAILED TO RECALL TO FIX THE PROBLEM. IT'S BRAKE ACTUATOR THAT FAILED AROUND 114,830 MILES. I REALLY DON'T KNOW WHAT TO DO NOW OTHER THAN TO TAKE THE CAR TO THE DEALER THIS SATURDAY.

TL* THE CONTACT OWNS A 2008 LEXUS LS460. THE CONTACT STATED THAT THE VEHICLE INDEPENDENTLY SURGED FORWARD AT A HIGH RATE OF SPEED WITHOUT WARNING. THE CONTACT APPLIED FORCE TO THE BRAKE PEDAL, BUT THE FAILURE RECURRED WITHOUT WARNING. WHILE AT A STOP WITH THE BRAKE PEDAL APPLIED, THE CONTACT'S VEHICLE SURGED FORWARD AND CRASHED INTO THE PRECEDING VEHICLE, WHICH THEN CRASHED INTO ANOTHER VEHICLE. THE VEHICLE WAS TOWED TO A REPAIR SHOP BY THE INSURANCE COMPANY WHO REPAIRED THE BODY DAMAGES TO THE FRONT OF THE VEHICLE.
