Consequence

If hot, pressurized coolant suddenly spills into the operator's foot area, there is an increased risk of a burn injury or a crash.

Remedy

Navistar will notify owners, and dealers will replace the formed heater hose with a more robust one, free of charge. The recall began on January 4, 2017. Owners may contact Navistar customer service at 1-800-448-7825. Navistar's number for this recall is 16510.

Consequence

If the bearings move out of position, the stowed platform may have excessive movement resulting in failure of the platform mounting. This failure could cause the wheel chair lift platform to fall out of the vehicle when the doors are opened, increasing the risk of injury for the lift operator.

Remedy

Navistar will notify the purchasers of the lifts, and dealers will inspect the link arms for damage or for displaced bearings. The platform will be inspected for cracks. Consequence

If the platform side plates crack, the lift platform can separate from the lift and come to rest against the vehicle's lift door. When the doors are opened, the platform may fall out, increasing the risk of injury to the lift operator.

Remedy

Navistar will notify owners to take buses to a Ricon dealer or service center who will install supplemental platform support bumpers. Consequence

If the latch plate remains partially engaged after the button is pressed, egress from the vehicle could be hindered which would increase the risk of injury in the event of an emergency.

Remedy

Navistar will notify owners, and dealers will inspect and replace the seat belt buckles, as necessary, free of charge. The recall began on February 26, 2014. Owners may contact Navistar at 1-800-448-7825. Navistar's numbers for this campaign are 13519 and 13520.

In the event of a crash, the seat padding may not properly protect against knee impact force, increasing the risk of injury.

Remedy

Navistar will notify owners, and dealers will install polystyrene blocks in the lower outside corners of each seat to increase the knee protection, free of charge. The recall began on October 10, 2013. Owners may contact Navistar at 1-800-448-7825. Navistar's recall campaign number is 13512.

Lap seat belts fail to latch

The Office of Defects Investigation (ODI) of the National Highway Traffic Administration (NHTSA), received a complaint from a school bus fleet alleging seat belt failures in their model year 2011 Navistar IC (Navistar) school buses, equipped with AmSafe non-retractable passenger two-point seat belts... . Following receipt of this complaint, ODI reached out to multiple school bus fleets across the country operating similar buses and confirmed the problem also existed in other buses.Following confirmation of similar seat belt failures in other school districts, NHTSA opened investigation (PE18-015) on December 17, 2018 to assess the manufacturer's data including warranty claims, field reports, design changes, etc.The scope of this investigation included all model year 2011 through 2016 Navistar school buses. Analysis of the manufacturer data provided to NHTSA in response to this investigation showed high seat belt failure rates in model year 2011 Navistar BE, CE and RE school buses when compared to peer vehicles. The elevated failure rates dropped on vehicles produced after two design changes made by the manufacturer during the production of 2011-2016 seat belts. Navistar implemented a dimensional change to the metal tongue on April 25, 2012 and started utilizing a more robust (car-type) seat belt starting April 3, 2015. The combination of these two changes significantly decreased the seat belt warranty rates in model year 2012-2016 buses. Navistar?s testing and engineering analysis showed that long term cycle fatigue (buckling and unbuckling the seat belt) can cause the plastic fingers attached to the metal tongue to crack and fall off. The missing fingers can result in an elevated force necessary to latch the tongue in the buckle.The elevated force required can prevent the buckle from latching, increasing the risk of an injury in the event of a crash. By a letter dated June 21, 2019 Navistar notified NHTSA of a safety defect in their model year 2011 Navistar IC BE, CE and RE school buses built with AmSafe non-retractable passenger two-point seat belts and initiated safety recall 19V-478. With recall action 19V-478 taken by Navistar this investigation is closed. Further use of agency resources does not appear to be warranted. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist on other model or MY vehicles outside of the recall scope. The agency reserves the right to take further action if warranted by the circumstances.Read more

Failed School Bus Seat Back Pans

On July 7, 2017, the Office of Defects Investigation (ODI) received a report from an executive officer of the State of Iowa's Bureau of Finance, Facilities, Operations & Transportation Services, informing ODI of numerous seat back pan weld failures (pan material fatiguing at weld points) in multiple... 2011 model year (MY) Navistar IC school buses.In August 2017, ODI engineers found similar failures when they visited an Arlington, Virginia public school facility to inspect buses ranging from MY 2011 to 2015. The first MY2011 bus inspected demonstrated the same issue (broken back seat pan welds) as reported on the Iowa buses. MY2012 - MY2015 buses did not demonstrate the failed seat back pan issue. NHTSA discovered that these buses had a different seat back pan design.ODI opened this investigation to better understand the potential risk of injury and evaluate passengercompartmentalization in the event of a crash involving a subject vehicle containing broken seat back pan welds. Navistar conducted sled testing at the Transportation Research Center of Ohio (TRC) to better understand if broken seat back pan welds could lead to a loss of passenger compartmentalization in a crash. ODI engineers were present and observed all tests done by the manufacturer.Although the test results indicated that passengers would remain compartmentalized, upon further review, ODI became concerned that the test manikin's necks seemed to over-extend when their heads struck the seat back frame and the torso pushed into the seat back pan. There was a concern that injuries may occur as a result of the broken seat back pan welds, allowing the passenger torso to push further into the seat back than if good welds were in place. Upon NHTSA's request, Navistar ran additional sled tests with instrumented test manikins to measure injury potential in the head, neck, and chest. The tests were conducted using Federal Motor Vehicle Standard (FMVSS) 208 as a guide and reference as there are not specific standardized tests for this particular alleged defect condition. NHTSA biomechanics and crashworthiness experts reviewed the results along with ODI and concluded that the test results were within the allowable threshold per the FMVSS.Navistar informed ODI that the subject vehicles of this investigation were also the subject of Technical Service Information document TSI-13-47-01 and recall 13V-308 and that during these campaigns involving seat backs and seat back foam covers, many of the vehicles had already had their seat back pan welds repaired. Navistar agreed to continue to support customers through extended warranties.Given the above facts and test results, ODI is closing the investigation as sled testing performed by the manufacturer has shown there appears to be little difference in the potential for injury between seat back pans with intact or broken welds. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist.
