Full Narrative
On June 13, 2013, at 2028 coordinated universal time (1528 local time), a Piper PA-22-135, Ecuadorian registration HC-CMX, was destroyed when it impacted terrain shortly after taking off from Taisha Airport (SETH), Taisha, Morona-Santiago, Ecuador, en route to Sucua Airport (SESC), Sucua, Morona-Santiago, Ecuador. The Ecuadorian commercial pilot and three passengers were fatally injured. The flight was conducted under Ecuadorian flight regulations.
