Full Narrative
On October 5, 2025, about 1310 central daylight time, a Piper PA-28-140, N4218T was substantially damaged when it was involved in an accident near McGregor, Minnesota. The pilot and two passengers were seriously injured. The airplane was operated under the provisions of Title 14 Code of Federal Regulations Part 91 as a personal flight.
The airplane departed from Isedor Iverson Airport (HZX) McGregor, Minnesota, and impacted a marsh field about .6 nm from the departure end of runway 14. According to law enforcement, the pilot said that the engine lost power and the airplane would not climb.
A postaccident inspection revealed that the fuel selector was set to the right fuel tank and there was fuel in both wing fuel tnaks. Fuel samples were taken from both fuel tanks, and no water was present in either sample.
The wings, fuselage, and empennage were substantially damaged. The airplane was retained for further examination.
