On July 14, 2016, about 0230 Coordinated Universal Time, an Aero Commander B1A airplane, C-GSQY, lost engine power and impacted terrain after takeoff near Hoey, Saskatchewan, Canada. The pilot sustained fatal injuries.

The Transportation Safety Board (TSB) of Canada is investigating the accident. As the state of manufacture of the airplane, the National Transportation Safety Board (NTSB) has designated a U.S. accredited representative to assist the TSB in its investigation.
