On April 11, 2016, about 2000 Coordinated Universal Time, a Rockwell Commander 112B airplane, CGQJN, impacted terrain near Revelstoke, British Columbia, Canada, under unknown circumstances. The pilot and passenger sustained fatal injuries.

The Transportation Safety Board (TSB) of Canada is investigating the accident. As the State of Manufacture of the airplane and engine, the NTSB has designated a U.S. accredited representative to assist the TSB in its investigation.
