On January 3, 2016, about 1730 Coordinated Universal Time (UTC), a Beech C90GTi, PP-LMM, registered to Bradesco Leasing S.A., operated by Supermercado Shibata Ltda, experienced controlled flight into mountainous terrain near Parati Airport (SDTK), Parati, State of Rio de Janeiro, Brazil. Instrument meteorological conditions prevailed in the area at the time of the accident and a visual flight rules flight plan was in affect for the non-U.S., non-commercial personal flight from Campo de Marte Airport (SBMT), Sao Paulo, State of Sao Paulo, Brazil, to SDTK, that originated about 55 minutes earlier. The airplane was destroyed and the commercial pilot-in-command and co-pilot were fatally injured.

The flight proceeded towards the destination airport with an IFR clearance with the intention of picking up a passenger, but cancelled IFR at some point of the flight and then activated a VFR flight plan. While at the destination airport which does not have a published instrument approach procedure, the flightcrew attempted 3 times to land with IFR conditions prevailing (witnesses reported low ceiling and visibility). During a go-around following the third attempt, the airplane crashed in mountainous terrain at about 1,700 feet elevation; the accident site was located about 4 nautical miles and 358 degrees from the destination airport.
