Incident Narrative
An employee was working to lower casting benches from the second floor to the first floor. The process involved several chain falls and a forklift. The employee was sitting over top of the floor opening when the bench rocked, causing him to lose balance and slide from the bench. He caught himself before falling to the floor below. The employee sustained injuries to his left shoulder, elbow, and hand. The employee was hospitalized and required surgery for his left forearm.
