Incident Narrative
An employee was conducting a visual inspection of the crane and rigging before hooking to a bundle of rebar. The employee attached the chain to the bundle and kept his hand on the hook and was using the remote control to raise the hoist. As the chain tightened, the employee looked up to ensure the hoisting block alignment. A chain crushed his right hand and fingers, causing closed non-displaced fractures to the last joints of the index and ring fingers. His ring fingertip was amputated.
