Incident Narrative

An employee was helping to install a storm drain. The process included digging a trench, laying the storm drain pipe, and burying the pipe. The employee climbed down inside the unguarded trench to get measurements and do leveling for the stabilizing sand. The employee started to prepare the sand so they could lay the pipe. An excavator put some sand into the trench. Dirt then fell on him resulting in fractures to an arm and a leg.
