Incident Narrative
An employee was holding down a bottom board at the conveyor while a block was being placed on the conveyor using a forklift. When the forklift pulled away from the block, the injured employee's left index finger was pinched between the wooden board that the block was resting on and the metal edge of the conveyor. The employee sustained a fingertip amputation.
