Incident Narrative
An employee was checking the tension of a belt on a fly ash auger that feeds into a concrete truck. The employee's fingers were caught in the belt pulley, resulting in hospitalization with fractures to their left index, middle, and ring fingers, as well as lacerations and tears to the skin/tissue of the fingers. The employee required surgery.
