Incident Narrative

An employee was operating a powered industrial tugger vehicle with a train of two racks behind it. The employee dismounted the tugger and went between the first and second racks to unhook the hitch joining the two racks and push the rear rack to the manufacturing line for delivery of parts. A forklift rear-ended the rear rack and the employee was pinned between the two racks. The employee suffered a left ankle fracture and a laceration below the back of the right knee, as well as multiple contusions and abrasions.
