Incident Narrative
An employee hooked a casing tool cradle to a loader, which then picked up the casing tool. The casing tool swung around and knocked the employee to the ground. The loader then went to set the casing tool on the ground and it struck the employee's left leg, resulting in a crush injury that required hospitalization and surgery.
