Incident Narrative
An employee was returning from break and walking past the meat department. She tripped on a stack of deck pans (that had been removed from meat case), causing them to fall. She then slipped on one of the pans and fell to the floor. The employee was hospitalized with fractures to her left shoulder, left arm, and a hip.
