Incident Narrative

On May 29, 2025, an employee was working at a blanker, putting tape on the male punch of the die to avoid jams and allow the blanks to be pushed out onto the cart. While conducting this operation, the machine actuated, resulting in injuries to four fingers on the employee's left hand. He sustained amputations of the index finger at the end of the nail, the middle finger at the top of the joint, the ring finger near the bottom of the joint, and the little finger near the bottom joint. The machine was not guarded at the time.
