Incident Narrative
On May 19, 2025, multiple employees were installing a 42-inch storm pipe in a trench that was 17 feet deep and 15 feet wide. The excavator operator was signaled to scoop soil from inside the trench to continue laying the pipe. As the operator went to remove the soil, the injured employee was pinned by the excavator bucket against the pipe, resulting in hospitalization for a fractured scapula and sternum.
