An excavator was hoisting a 4,400-pound trench ditch box, suspended from its hook by manhole chains on a trackhoe bucket. The chains slipped and the ditch box fell onto an employee, causing him to fall to the ground. He was then trapped under the spreaders of the ditch box. He suffered a broken right leg and was hospitalized.
