An employee was using a rag to wipe grease off a chain on a thermal forming machine. The rag caught on a sprocket and pulled the employee's left hand into a pinch point, resulting in a soft tissue avulsion of the little fingertip and fractures in the index and ringer fingers. The machine was in operation at the time of the incident.
