Incident Narrative
An employee was hydro-blasting the walls inside a tank to remove polymer buildup. The weep hole on the inlet fitting of the hydro-blasting shotgun leaked, causing a high-pressure water injection to the employee's abdomen. The employee was hospitalized with an internal injury requiring surgery.
