Incident Narrative
On April 14, 2025, employees were on a roof putting metal decking on the roof. Their fall protection (yo-yo lanyards) became entangled. One employee disconnected the cord to untangle the lines and he fell approximately 10 feet from the lowest point of the roof to the ground. The employee sustained two fractured ribs and bruising to his side.
