Incident Narrative

At 3:47 p.m. on April 9, 2025, an employee was about to fill a tote with water on the back of a flatbed truck. When the water pump was turned on, the hose the employee was holding surged out of his hands. He lost his balance and fell to the ground 41 inches below the truck bed. His head struck the ground and he went unconscious and had a large bump on right side of his head about 1-2" above his ear. The employee was hospitalized.
