Incident Narrative

An employee was completing a bevel at the end of a steel pipe. The next piece of pipe to be beveled in the beveling machine was not lined up. The employee used his hand to stabilize the unaligned piece of pipe, when the pneumatic pipe clamp came down and crushed his right arm. The employee sustained fractures requiring surgery.
