Incident Narrative
An employee was using a dolly to move a full barrel of SLS liquid from the conference room where it was being warmed, to the production floor. The barrel slipped out of the dolly and began falling. The employee went to grab the barrel and their finger was caught between the barrel and the floor. The employee's fingertip was amputated.
