Incident Narrative
An employee was pushing a roller-based tabletop cart (with an electric bagging unit on top) forward from one location to another. As the wheels rolled over a concrete seam in the floor, the front wheels caught in the seam and the cart tipped over forward. The front of the cart rolled backward toward the employee, and the front of the table (or its frame) landed on the employee's right foot. The big toe and the second toe were broken.
