Incident Narrative
At 1:25 p.m. on March 13, 2025, a Michels Pacific Energy (MPE) crew was terminating new wires connecting a new three-phase sector and an existing pad-mounted switch elbow connected switch gear 20 feet away. The circuit being worked on was de-energized and isolated. All other circuits in the switch gear were insulated with insulating caps. After termination of the elbow, a strand of the new concentric neutral wire, used for bleeding capacitance, moved and made contact with the insulated bushing cover in the switch gear. The concentric neutral became energized from induced current at an unknown voltage. An employee working in the new sector was shocked on the right hand.
