Incident Narrative

An employee was replacing the pull mechanism (rope) on a dunk tank when he slipped from a fixed folding chair mounted on the floor, causing his right little finger to become caught on the metal corner of the water tank. He sustained amputation of the right little finger to the second knuckle.
